Physicians refer patients who display one or more of the following conditions:

• Dysphonia

• Dysphagia

• Chronic cough

• Gender dysphoria related to voice

• Paradoxical vocal fold motion (PVFM)/Vocal cord dysfunction (VCD)

• Exercise-Induced Laryngeal Obstruction (EILO)

• Pre-radiation/chemo for head & neck cancer

• Dysarthria

• Aphasia

• Traumatic Brain Injury

• Concussion

• Long-COVID

• Chemobrain

• Cognitive-communication disorder

If you are wanting to utilize insurance for coverage of your sessions, you will need to have a referral from a provider. If you are planning to do private pay, you do not need a referral from a physician.

Physicians must complete a prescription or referral form. The prescription or referral form must specify: speech-language pathology evaluation and treatment with diagnosis and ICD-10 code(s). Referrals can be faxed to Empowered Voice Rehabilitation at (904) 395-2255.

Empowered Voice Rehabilitation will complete an assessment and prepare a recommended plan of care for review and approval by the prescribing physician/medical provider.

Treatment will be completed when physician approval has been received.
