Vocal Health & Hygiene Questionnaire (Digital Download)
Purpose of This Resource
The Vocal Health & Hygiene Questionnaire is a comprehensive way to help clinicians to better understand vocal hygiene practices of their clients. This questionnaire is designed to help speech-language pathologists (SLPs) identify behavioral, medical, dietary, environmental, and phonotraumatic factors that may contribute to voice disorders or limit recovery. It supports holistic, individualized voice therapy planning by bringing clarity to lifestyle influences and potential risk factors or patterns.
This 7-page digital download comes with a 4-page checklist that allows the clinician to examine factors related to General Health/Medications, Phonotrauma, Environment, Dietary Factors, Estimated Voice Use, and Functional Impact for each client. It also comes with examples of how to translate questionnaire findings into clinical documentation for SOAP Notes and SMART Goal development.
Why It Matters
Voice disorders rarely occur in isolation. This tool allows clinicians to assess the everyday realities of a client’s life. These causal and contributing factors often play a significant role in the onset, persistence, or recurrence of dysphonia.
It also empowers clients to reflect on how their behaviors and routines affect their voice health, promoting buy-in and self-awareness as part of the therapeutic process.
What’s Included
A patient-reported questionnaire evaluates five key areas:
General Health & Medicine
Environmental Factors
Phonotraumatic Behaviors
Dietary Irritants
Estimated Voice Use
A functional impact scale is also included, allowing patients to self-report how their voice affects daily life, which can guide therapy goals and progress tracking.
Clinical documentation and SMART goal examples that show how to translate questionnaire findings into concise, functional documentation suitable for the medical record
How to Use This Tool
Use during initial evaluation or early treatment sessions to screen for possible causal and/or contributing vocal hygiene factors and establish a baseline.
Identify modifiable risk behaviors and guide early education in therapy.
Refer to the data when customizing home programs, hygiene counseling, and collaboration with medical providers (e.g., pulmonology, ENT, nutrition).
Re-administer during therapy to monitor change or reinforce behavior modification.
Important Note
This questionnaire is a clinical support tool, not a diagnostic instrument. While it highlights likely voice risk factors, final interpretation should always be integrated with instrumental assessment, patient-reported symptoms, and professional clinical judgment.
This is a digital download, so you get a download link emailed to your inbox instantly. You can use it right away electronically and you can print off and laminate for use with your patients.
